AthleticsMD LLC · DBA Prehab Centers of America
Physician-owned · Columbia & Laurel, Maryland · Telehealth statewide
Effective August 1, 2025 · Last revised February 7, 2026
AthleticsMD LLC, doing business as Prehab Centers of America, is a physician-owned medical practice. We bill for professional physician-office and clinical services: office visits, consultations, evaluations, care coordination, diagnostics, and enrollment in clinical programs such as ShredRx™ medical weight management and PREOOP™ surgical prehabilitation.
AthleticsMD LLC is the merchant of record. Card payments are processed by Stripe.
Your bank or card statement will read AMD DBA PREHABAMERICA.
If you see that descriptor and are unsure what it is, it is us. Please call 844-348-6348 or email care@athleticsmd.org and we will identify the charge for you immediately.
AthleticsMD is responsible for refunds, billing questions and dispute responses on those professional-service charges.
Your first physician consultation is $0. It is genuinely free: no fee, no enrollment, and no purchase is required to attend it, whether you are seen in Columbia, in Laurel, or by Video telehealth anywhere in Maryland.
Charges arise only after that, and only for what you agree to:
We do not add charges you have not seen and agreed to in advance.
When you book, we ask for a card to confirm the appointment. Nothing is charged at booking. The card is stored securely by Stripe as a card on file — we never see or hold the number. A physician's hour is blocked for you, your history is reviewed in advance, and that time cannot be given to another patient. The card exists so that a held hour means something.
Tell us and there is no charge. Ever. Call or text 844-348-6348, or email care@athleticsmd.org, any time before your appointment — however close to it you are — and we will move or cancel it at no cost. No fuss and no questions asked. There is no cancellation fee at AthleticsMD.
| What happened | What you are charged |
|---|---|
| You attended | $0 |
| You cancelled or rescheduled, at any notice | $0 |
| You did not attend and did not tell us | $25.00 |
The $25 no-show fee is charged to the card on file only when an appointment is missed without any notice. It is disclosed on the booking screen, in writing, before you enter your card, and it is the only charge that can reach that card without a further step from you.
It is not charged straight away: there is a short grace period of about 2 hours after the missed time, so if something happened you can simply call us first and nothing is taken.
We waive it in the circumstances you would expect — a medical emergency, a hospitalisation, a bereavement, a genuine mix-up on our side. Ask us, at any point: if it has already been charged we refund it to the same card. And whether or not you are charged, we will still follow up to get you rescheduled: the fee is not a door closing.
Repeatedly missing appointments without notice may lead us to ask for a prepaid visit before booking further time, or to decline further online self-booking.
ShredRx™ is offered on published terms, which are shown to you before you enrol:
| Option | Price | How it behaves |
|---|---|---|
| Three months prepaid | $99 / week | Locked. The price does not move when your dose increases. |
| Month to month | $129 / week | Steps to $139, then $149 as your dose increases. Never exceeds $149. |
Where a program is billed on a schedule, the authorisation you accept at checkout states the amount, the interval and how to stop it. You may cancel future billing at any time by contacting us at 844-348-6348 or care@athleticsmd.org. Cancellation stops future charges; it does not reverse periods already delivered.
Promotional pricing and discounts — including any first-visit or first-months discount offered to you — reduce what you pay. They do not otherwise change these terms, and a discounted enrolment is refunded on the amount actually paid.
Fees for professional services that have been rendered are generally not refundable. Clinical time, physician review, evaluation and clinical judgement are delivered when they happen and cannot be returned. This is standard for medical practices.
Email care@athleticsmd.org or call 844-348-6348 with the date and amount of the charge. We acknowledge refund requests within two business days and aim to resolve them within ten business days. Approved refunds are returned to the original payment method; the time it takes to appear on your statement is set by your bank, typically five to ten business days.
Please raise a refund request within 60 days of the charge, while records are current. We will still look at older requests, but we may be limited in what we can adjust.
Medicine does not come with guaranteed outcomes, and no refund here is contingent on a clinical result. Enrolment buys physician care — evaluation, prescribing where clinically appropriate, monitoring and adjustment — not a specific amount of weight lost or any other promised endpoint. A physician may also determine that a treatment is not clinically appropriate for you; in that case you are not charged for treatment you do not receive.
Please talk to us first. Nearly every disputed charge we see is a question, not a disagreement: an unfamiliar statement descriptor, a forgotten appointment, a family member's visit, or a no-show fee whose reason was never explained. One phone call resolves those in minutes. A chargeback takes weeks, and if the underlying issue was one we would simply have refunded, the dispute costs us both time for no gain.
Call 844-348-6348 or email care@athleticsmd.org before filing with your bank. If we owe you a refund, you will get it without needing your bank to ask.
We respond to every dispute with the documentation we hold, which for a typical charge includes: the booking record and the time it was made; the policy text displayed on screen at the moment of authorisation, including the no-show fee; the payment authorisation and its timestamp; the appointment and attendance record; and the messages exchanged with you. This page, as published on its effective date, forms part of that record.
Where a dispute is resolved in our favour, the original balance remains payable, and any fee charged to us by the card network may be added to your account. An account left unpaid after a dispute concludes may be referred for collection.
None of that is adversarial by intent. We would far rather refund a patient who has a fair point than win an argument with one.
If you believe your card was used at AthleticsMD without your permission, contact us immediately at 844-348-6348. We investigate promptly and refund confirmed unauthorised charges in full.
When one of our licensed clinicians issues a prescription, it is transmitted to a pharmacy you select — CVS, Walgreens, another community pharmacy, or a legally licensed compounding pharmacy.
AthleticsMD does not sell you medication and does not bill you for it. What you pay the pharmacy is set by that pharmacy and, where applicable, your insurance or discount plan. Billing, fulfilment, shipping and refund questions on a filled prescription are handled by the pharmacy under its own policies, not by us.
Our fees cover professional clinical care: the evaluation, the prescribing decision, the monitoring and the dose adjustment. Separately, and only for lawful in-office use in the course of clinical care, AthleticsMD may purchase medications directly.
Where a service is billed to insurance, your responsibility is determined by your plan — deductible, co-insurance, co-payment and coverage decisions are set by your insurer, not by us. If an insurer later reprocesses a claim and it turns out you overpaid, we refund the overpayment. Charges for services your plan does not cover remain your responsibility, and we will tell you in advance wherever we can.
Billing, refunds and disputes:
This policy is provided to patients on request, alongside invoices and checkout receipts, and at this address. It has been in effect since August 1, 2025 and was last revised February 7, 2026. We may update it; the dates above always show the current version, and the version in force when you were charged is the one that governs that charge. Material changes do not apply retroactively to charges already made.
This policy is governed by the laws of the State of Maryland. Nothing here limits any right you hold under applicable federal or Maryland law, or under the rules of your card network.
Clinical and medical-records questions are handled separately from billing — call 844-348-6348 and ask for the clinical team. For a medical emergency, call 911.
